Q1Group 2016 Medicare drug plan Analysis: Over 10 million Medicare beneficiaries will see at least 100 drugs removed from their 2016 Medicare Part D drug plan formularies

With each new plan year - and throughout the plan year - your Medicare Part D plan (PDP) or Medicare Advantage plan that provides prescription coverage (MAPD) can change their formulary or drug list, introducing or removing medications.

As a note: The size of a Medicare Part D prescription drug plan formulary is measured in NDCs or National Drug Codes.  These 11 digit codes are used by doctors and pharmacies to uniquely identify a medication, the strength, and the packaging.  The NDC for your medication appears on the label of your prescription bottle. (Throughout this article, we use the terms drugs, medications and NDCs interchangeably.)

For the 2016 plan year, the largest increase in stand-alone prescription drug plan formulary size is the EnvisionRxPlus Clear Choice plan in South Carolina, Oregon and Washington State (members in other states are being reassigned to the EnvisionRxPlus Silver plan).  The 2015 EnvisionRxPlus Clear Choice plan formulary contained 3,212 drugs and the 2016 formulary will contain 3,355 drugs – an addition of 143 medications or NDCs.

However, the vast majority of stand-alone Medicare Part D prescription drug plans will see a reduction of at least 100 medications from their current plan formulary.  In fact, based on our analysis, over 10 million Medicare beneficiaries currently enrolled in stand alone Medicare Part D plans that will see at least 100 NDCs removed from their formulary.  For example, the Pennsylvania and West Virginia 2016 Blue Rx Complete (PDP) will lose 1,351 drugs as compared to the plan's 2015 formulary.

Changes in the largest 2015 Medicare Part D formularies
Here are several examples of 2015 stand-alone Medicare Part D plans with the largest formularies and how their formularies are changing in 2016.  (Again, the formulary size in the chart below is measured in NDCs or National Drug Codes -- used by pharmacies to uniquely identify a medication, the strength, and the packaging).

Although three of the largest 2016 drug plan formularies shown in the table above are offered by "Blue" plans, several of the 2016 Medicare Part D plans with the smallest formularies are also Blue plans.

Please remember: A “larger” or “smaller” formulary does not necessarily reflect on the quality of the Medicare plan. Your goal is to find a Medicare plan that offers the most affordable coverage for your 2016 health and prescription needs.

Browsing any Medicare plan’s formulary:
Our Formulary Browser for both stand-alone Medicare Part D plans and Medicare Advantage plans can be found at Formulary-Browser.com/2016. Here, you can browse by letter though each plan’s entire formulary to review cost-sharing and the details of the drug’s utilization management restrictions such as quantity limits, prior authorization, and step therapy. Quantity limits are displayed as Q:30/90days meaning that this drug has a quantity limit of 30 units (such as “tablets”) for a 90 day period.

Comparing drug costs across Medicare plans:

  • Medicare MSA Plans combine a high deductible Medicare Advantage Plan and a trust or custodial savings account (as defined and/or approved by the IRS). The plan deposits money from Medicare into the account. You can use this money to pay for your health care costs, but only Medicare-covered expenses count toward your deductible. The amount deposited is usually less than your deductible amount, so you generally have to pay out-of-pocket before your coverage begins.
  • Medicare MSA Plans do not cover prescription drugs. If you join a Medicare MSA Plan, you can also join any separate (stand-alone) Medicare Part D prescription drug plan
  • There are additional restrictions to join an MSA plan, and enrollment is generally for a full calendar year unless you meet certain exceptions. Those who disenroll during the calendar year will owe a portion of the account deposit back to the plan. Contact the plan provider for additional information.
  • Medicare beneficiaries may enroll through the CMS Medicare Online Enrollment Center located at www.medicare.gov.
  • Medicare beneficiaries can file a complaint with the Centers for Medicare & Medicaid Services by calling 1-800-MEDICARE 24 hours a day/7 days or using the medicare.gov site. Beneficiaries can appoint a representative by submitting CMS Form-1696.
