1. Beneficiary pays a $265 Deductible
2. Beneficiary pays a 25% of Costs between $266-$2400
3. Beneficiary pays a 100% of Costs between $2400-$5451.25

Your annual out-of-pocket costs are estimated to be:

If your annual prescription costs are over $5451.25: On the costs under $5451.25, you pay as noted above (up to $3850), on the costs over $5451.25, you pay (a) the greater of either 5% of the prescription drug costs or $2.15 for generic prescription drugs and (b) $5.35 for brand-name prescription drugs. Medicare Part D pays the lesser of either: 95% of the prescription drug costs or the balance of the prescription drug costs.

The Annual Out-of-Pocket cost above represents your costs in addition to paying your plan premium. This calculator is based on the guidelines provided by Medicare as of August, 2006.
