The list below was released by the Centers for Medicare and Medicaid Services (CMS) and shows the company's parent name and the name used by the company when offering their prescription drug plans (PDPs).

Employer sponsored plans (800 series) are excluded.
National is defined as plans offered in all 50 states plus Washington, D.C.
